All employees of Safe Children Coalition agencies and contracted vendors are to comply with the CBC Incident Reporting Procedure, for incidents involving CBC employees and clients. The purpose of the incident reporting system is to provide a structured protocol for identifying and reporting incidents in order to ensure safety, mitigate risk, address high risk factors, comply with DCF reporting requirements, and ensure appropriate handling of situations notifications and follow up actions to protect both staff and clients. The Incident Report does not replace existing abuse, neglect and/or exploitation reporting requirements through the Florida Abuse Hotline at 1-800-96ABUSE (1-800-962-2873).
Upon becoming aware of an incident, the Child Welfare Professional is responsible for the fully completed, approved, and signed Incident Report Form being submitted within 12 hours of a critical incident occurring or learning of the incident OR within 24 hours of a non-critical incident occurring or notification. Incidents should be reported to the agency supervisor immediately upon receiving information. The incident shall be reviewed with the Supervisor and recommendations to ensure child safety shall be discussed with the Child Welfare Professional. The employee shall follow through on all recommendations immediately.
Instructions for completing the Incident Report Form are included in the word document on pages below the fillable form. The Incident Report Form should be filled out completely with as much detail regarding the incident as possible. When the incident involves a parent or caregiver, ensure the child’s name is included in the report. The IR Form should be saved in the following format: client’s first initial, last name, date of incident and county from which child is receiving primary case management services (example: A. Smith 05-05-26 Sarasota). The IR Form is transmitted via email distribution list as detailed in the Incident Report Form instructions.
Information can be updated in a subsequent report if additional information is obtained (use the follow up only check box). All Incident Report Forms must be submitted by a supervisor or above. Case Managers are responsible for entering an FSFN note documenting the completion of the Incident Report, details of the incident, notifications, and needed follow up within 2 business days of incident report.
